In your thirties, you begin to pay closer attention to your skin. Some time has passed since you fretted over every blemish and any sign of a breakout. The Patient

This patient was exhibiting the early signs of aging typical of a thirty-something.  She started to develop smile lines around her mouth and some crow’s feet beside her eyes which made her look tired.  The combination of Ultherapy and a Flash Lift rejuvenated her appearance and gave her a refreshed, youthful look. 

Ultherapy

Ultherapy uses micro-focused ultrasound technology to tighten skin and boost your body’s own collagen production. It is a non-invasive procedure that can be used to treat wrinkles and sagging skin in the brow area, midface, chin, and neckline. While there is some discomfort involved, this is considered a wonder of modern medicine that allows patients to get tighter, firmer skin without any downtime.

Flash Lift (PDO Thread Lift)

A Flash Lift is a specialized thread lift developed by Wave Plastic Surgery.  This thread lift uses threads made of polydioxanone (also known as PDO) which are expertly placed in the skin to provide a natural tightening and lifting effect.

Something happens when a person reaches their forties. Their skin can lose its natural glow and begin to look dull and feel dry. Sunspots and wrinkles develop with more frequency and the firmness of youth can begin to droop into sagging skin. In women, declining estrogen levels can emphasize these changes.

Women in their forties begin to experience perimenopause as they transition into menopause. During perimenopause, hormonal changes lead to diminishing collagen production. This can result in hooding of the eyelids, under-eye bags, and sagging skin on the face and neck.

These changes to your hormones and metabolism aren’t easily fixed with lifestyle changes and topical creams.  To get to the root cause of these signs of aging a person has to go deeper to address the underlying cause of these symptoms.

The Patient

This patient was exhibiting the early signs of aging typical of a forty-year-old.  She was beginning to develop under-eye bags which made her look tired.  Smile lines were forming in the nasolabial folds beside her mouth.  And she was experiencing the early signs of volume loss which made her facial skin less firm and toned. 

This patient wanted to address the signs of aging before they developed any further. To enhance her appearance and maintain her radiant complexion she opted for a combination of an Endoscopic Brow Lift, an Endoscopic Facelift (Wave Lift), Upper Eyelid Surgery, and Lower Eyelid Surgery.

Endoscopic Brow Lift

An Endoscopic Brow Lift uses a minimally invasive, endoscopic technique that involves using a tiny camera to allow surgeons to see inside of a person without the need for open surgery.  The purpose of a brow lift is to get rid of forehead wrinkles and drooping upper eyelids.  This is accomplished by repositioning the underlying tissue and muscles, which results in a more youthful appearance in the upper third of the face.

Endoscopic Facelift (Wave Lift)

The Wave Lift is a procedure exclusively developed by Wave Plastic Surgery to address the early signs of aging in the midface and lower face.  It is designed to be the gold standard of minimally invasive anti-aging surgery.  The Wave Lift produces dramatic results with minimal recovery time.  An Endoscopic facelift uses a similar technique to the Endoscopic Brow Lift mentioned above.  Only the endoscopic facelift is used to treat the signs of aging in the midface, chin, and jawline.

Upper Eyelid Surgery

Upper Eyelid Surgery or Upper Blepharoplasty can be used to combat the signs of aging by removing excess skin and fat deposits from the upper eyelid to correct drooping eyelids.  During this procedure, a surgeon will make a discreet incision along the natural folds of your upper eyelid skin. From here, they can carefully remove any excess skin or tissue.

Lower Eyelid Surgery

A common sign of aging is the development of wrinkles and bags under the eyes.  This can make you look tired or displeased.  Because the eyelids are extremely delicate and susceptible to the forces of gravity, the lower eyelids are one of the first areas of your face to show the signs of aging.  Lower Eyelid Surgery can be performed to remove excess skin and fat, and in some cases, repositions fat to create a refreshed and rested appearance.

As the aging process continues into your fifties the slowing of collagen production continues to be an issue. Around age 50, women start experiencing menopause which causes a women’s ovaries to produce less estrogen and progesterone.

Estrogen helps your body hold in moisture, keeps the skin barrier strong, and promotes collagen production. A change in estrogen levels often causes the skin to become dry and more easily irritated. The loss of collagen also leads to sagging skin along the jaw and around the eyes.  Without collagen to keep the skin in a firm and taught position, it begins to deflate and droop.

At this point in the aging process, the fat and tissue deposits beneath the facial skin have lost volume.  In order to counteract the signs of aging, a plastic surgeon must address the superficial muscular aponeurotic system or SMAS.  This is the layer of tissue deep within the skin and subcutaneous tissue that supports the structures for the face.

This can be done with a High SMAS Full Face and Neck Lift.  The patient below also opted to undergo the complimentary procedures of an Endoscopic Brow Lift to address the upper third of her face and a Chin Augmentation to achieve a dramatic, full face rejuvenation.

The Patient

This patient was exhibiting the typical signs of aging for a person in their mid-fifties.  She had pronounced nasolabial folds (smile lines) and had some hollowness and discoloration under her eyes.  She also had some sagging in the chin and neckline where jowls were beginning to form. 

The procedures outlined above were able to give her a more youthful, rejuvenated appearance with a firm jawline and a pronounced chin.

High SMAS Full Face and Neck Lift

A High Extended SMAS Face and Neck Lift addresses the lower two-thirds of the face and neck.  The goal of this type of facelift is to restore the facial tissue to its previous anatomical position.  To achieve lasting results, this tissue must be lifted by elevating the layer of sub-dermal tissue called the SMAS/Platysma.  When this is tightened it can re-elevate the facial structures and reposition the tissues to a more youthful location.

Chin Augmentation

Chin augmentation surgery, or genioplasty, uses a customized chin implant to strengthen a weak or recessed chin which subsequently enhances the contours of the jaw, neck,  balancing the overall facial profile.  The ideal chin is thought to be one that extends to or slightly behind a vertical line dropped from the forward most point of the lower lip on profile view.

Many people in their sixties have adult children who have left home, and they may be considering retirement or at least winding down their careers.  They may be planning on traveling or plotting out ways to spend their newfound free time.   In many ways, your sixties should be a rebirth, a time to fill your life with new experiences.  But people in their sixties may feel like their appearance doesn’t match their fresh outlook.

In order to achieve a look that matches your new lifestyle, you may want to reverse the effects of aging.  People in their sixties often experience thinning skin.  Sometimes called ‘crepey,’ your skin may look fragile and finely wrinkled like crepe paper.  While you may pine for the skin of your youth, your skin may not be what you should focus on.  Many of the most visible signs of aging are a result of volume loss underneath the skin. 

As your facial skin ages, it loses elasticity.  This is true for both the exterior epidermis as well as the SMAS membrane beneath the skin. This can cause sagging cheeks, jowls, a turkey neck, or a double chin.   It will also exacerbate the appearance of smile lines beside the mouth and drooping upper eyelids.  

The patient below received a High SMAS Full Face and Neck Lift and an Endoscopic Brow Lift as describes above.  She also opted for a concurrent Double Pure Fat Transfer to re-volumize her face to ensure that it would remain smooth, toned, and glowing with sufficient tissue volume for years to come.

The Patient

This patient showed the signs of aging typical in a sixty-year-old woman.  She had sagging skin along her jawline that had begun to form jowls.  She had developed excess skin on the front of her neck and had defined nasolabial folds running from the sides of her nose down to her chin.

Not satisfied with her appearance, she opted for a selection of three complementary cosmetic procedures.  The endoscopic Brow Lift would address the wrinkling on her forehead and drooping upper eyelids.  The High Extended SMAS Facelift and Neck Lift would address the sagging skin on her midface, jawline, and neck.  And the Double Pure Fat Transfer would replenish the lost volume and give her face a smooth, firm appearance.

Time passes more quickly as we age.  So many people are caught off guard when their seventieth birthday rolls around.  Yes, your body is in its seventies but all the preconceived notions that you may have had about this time in your life don’t necessarily hold true.  You still feel vibrant and energetic, but the face looking back at you in the mirror may give off a different impression.  

After turning twenty a person produces 1% less collagen than the preceding year.   So by the time you reach your seventies, your collagen levels have diminished significantly.  Subcutaneous fat also decreases which can contribute to sagging skin and age lines, especially on the face, arms, and hands.

The Patient

This patient is a woman in her seventies who exhibited the typical signs of aging.  Similar to the woman shown above, she had forehead wrinkles and a sagging brow in the upper third of her face. Volume loss to her midface gave her under-eye-area a hollow, sunken look.  The sagging skin on her cheeks gave her more pronounced smile lines and her neck and jawline were drooping with excess skin.  

Each of these issues were able to be corrected with a specialized procedure tailored to each specific area of her face, which worked in concert to achieve a dramatic transformation.
