Subscription Program: Terms & Conditions

  • Requires a 12-month commitment and will renew automatically after the initial 12 months.
  • Subscribers may choose to make a one-time payment or monthly payment for the full year subscription fee.
  • Subscriber on a monthly payment plan shall immediately notify us with a new payment method if any changes in Subscriber’s monthly payment method.
  • The yearly subscription fees are not refundable. The monthly paid Subscribers are responsible for the full payment of the yearly Subscriber fee upon cancellation at any time prior to twelve months period.
  • Subscribers may cancel by a written request. Credit card and account information will be securely kept on file, along with your signed authorization form indicating acceptance of subscription membership charges.
  • Rewards may not be combined with any other specials or promotions and may not be redeemed for cash.
  • Subscriber benefits are only valid for the registered subscriber and are not transferrable.
  • Pre-purchasing treatments prior to the expiration of subscription are prohibited.
  • There are no refunds of paid membership fees or unused treatments.
  • Subscriber may upgrade or downgrade her/his subscription-only after the expiration of the 12-month commitment.
  • Subscriber may use Wave Plastic Surgery’s gift/s only within the last month of the 12-month subscription commitment.
  • Subscription benefits must be used during the enrolled 12 months and the benefits will not be saved or rollover.
  • Wave Plastic Surgery reserves the right to change services and pricing at any time.
  • Wave Plastic Surgery reserves the right to refuse or discontinue services for any reason.
  • All sales are final.

To ensure the continued health of our clients, our team, and the community during this unprecedented time, we are adhering to strict social distancing rules and following CDC guidelines for COVID-19 to provide you with a clean, safe, and comfortable experience. Please keep in mind that when booking your virtual consultation, our team will kindly ask for you to consent to the following protocols:

All patients who visit our locations are required to undergo a COVID-19 screening. For more detail please call our office at 888-674-3001.

For surgical/ non-surgical virtual consultation, a $100 non-refundable consultation fee is required to secure your virtual consultation. If you wish to proceed, the consultation fee will be applied to your treatment.

For surgical/ non-surgical in-person consultation, we will require: 

o   $100 non-refundable consultation fee with our consultant/ mid-level providers; 

o   $250 non-refundable consultation fee with our plastic surgeons; 

o   $500 non-refundable consultation fee with Dr. Peter Lee; 

o   This fee is non-refundable but can be used as a credit for future treatments or retail product purchases.

Please arrive 30 minutes prior to your scheduled appointment to accommodate for additional screenings. We offer a 15-minute grace period, please note that if you are more than 15 minutes late, your treatment may be shortened or rescheduled. In addition, you will also be charged a $100 late fee.
